Please note due to rising costs for all aspects of the conference we have had to increase our registration fees for the first time since 2017.  Additionally, we are charging a nominal fee for the President's reception this year to discourage no shows and the resulting wastage.  Our registration fees include coffee and tea at breaks, one alcoholic beverage and appetizers at the Exhibitor's reception on Friday evening, breakfast every morning and lunch on the Friday and Saturday of the conference.   Each half day workshop ticket includes one meal and beverages at the break.  If you have any trouble with this form or have any questions about registering for this event, please contact registration@transfusion.ca

Cancellation Policy
 
Notice of cancellation must be made in writing to the Conference Secretariat at registration@transfusion.ca . A $60.00 (plus 14.975% QST + GST) cancellation fee will apply for conference registrations cancelled until May 19, 2023. After this date, no refunds will be issued. 

Registration Policies

  1. Registration fees must be paid in full before you will be admitted to the conference. All registration fees are subject to 14.975% QST + GST. All fees are listed and must be paid in Canadian dollars (CAD)
  2. Substitutions may be made until May 27, 2023 by writing to the Conference Coordinator at registration@transfusion.ca.
  3. Registration fees must be paid by VISA or MasterCard.90
  4. A confirmation notice detailing your registration will be sent to you immediately upon completion of this registration form. If you do not receive this confirmation within 24 hours, please contact us at registration@transfusion.ca.
  5. Recording may occur through the conference; by agreeing to these policies, conference attendees consent to filming, photography and sound recording as delegates, speakers or exhibitors at the conference.
  6. All registration rates are subject to availability.
  7. Completion of this form constitutes a binding agreement to the policies listed above.

Member Registration Codes

CSTM members are required to enter the membership discount code in the "discount code" box at the bottom right of the form and click on the green "apply" button in order to receive the membership rate listed above. The membership code was provided to all active members on January 23, 2023.   For those who obtained their membership after January 23, 2023 the code will be sent with your membership confirmation.   

 If you need to renew or sign up for new membership, please visit the CSTM website at www.transfusion.ca ; alternatively you can bundle your membership with member rate full conference registration by selecting the Full Scientific Program plus CSTM Membership ticket.

 
Students

To receive the student rate, proof of full-time enrolment must be provided. A valid student card or a copy of current course schedule will be accepted. This can be emailed to registration@transfusion.ca . Upon receipt of proof of enrollment, a discount code will be provided to reduce the regular rates to student rates. Do not register until you have received the discount codes.
